Forms
After enrollment it is possible to request the permits needed to access the Department of Physics grounds and its laboratories, using the following forms:
- Entry Authorization Form: IT – EN
- Entry Badge Request Form: IT – EN (Note: Fill in the assigned room number. If unavailable, ask your Supervisor, who will forward the request to the Area Manager.)
- Risk Assessment Form [IT]
- Study Plan Submission [IT]
By December of the enrollment year, the student will be required to submit the Study Plan [IT].
Period of Study or Research Abroad
To spend a period of study or research at a foreign University or Institution, at least 20 days before departure it is necessary to inform the following contacts, attaching the following forms:
- Invitation letter from the Host Organization;
- Signed Educational Program [IT];
- Supervisor declaration concerning the laboratory activities and the related risks, if any;
- Request for Scholarship Increase [IT].
At the end of the period abroad it is necessary to submit a Final Declaration signed by the Host Organization (the same used for the scholarship increase request).

Period of Study or Research at an Italian or Foreign Company or Business
For a period of study or research at a Private Company or Business it will be necessary to start a partnership or obtain a Declaration of Commitment, so at least three months before the departure date it is necessary to submit to the contacts below the following forms:
- Signed Educational Program [IT];
- Supervisor declaration concerning the laboratory activities and the related risks, if any;
- If abroad, a Request for Scholarship Increase [IT].
At the end of the period abroad it is necessary to submit a Final Declaration signed by the Host Organization.
